Valley Eldercare Center

Setting

Valley Eldercare Center is a 160 bed skilled nursing facility, providing 24-hour nursing care by licensed nursing staff who are trained for most sub-acute needs. Our nursing home is located at 2900 14th Avenue South, Grand Forks, ND at the south end of the medical park complex.

The 160 bed facility was built in 1981 in a unique, innovative design with five courtyards that promotes a warm caring environment. Forty eight beds are semi-private rooms and one hundred and two are private rooms.

Care

Here at Valley Eldercare Center, we strive to maintain an environment that accommodates the various levels of care needed by each individual. All of our 160 beds are Medicare/Medicaid certified meeting the needs of those that qualify to the criteria. The staff of Valley Eldercare Center is trained to handle a broad scope of care needs of the residents that we serve. Our goal is to maximize the life of all who live here.



Rehabilitative Therapies

Physical, Occupational and Speech therapies are provided for residents with a physician's order. The therapy staff works with each resident to assist them to reach the highest possible level of independence. The resident's therapy goals are often times met, and we have a history of discharging half of the residents that we serve to a lower level of care whether it is their home, independent living center, assisted living center, or apartments.

Recreation Programming

The Recreational Services Department provides a variety of activities which may include: musical groups, sing-a-longs, support groups, current events, games, bingo, floor hockey, field trips, etc. Special groups are also brought into the facility to provide various forms of entertainment. The department also provides a number of personal activities to individuals unable to take part in group activities. The goal of the department is to strive through its programming to enhance the quality of life for each resident and to foster healthy social relationships among the residents.

Dietary Services

The Dietary Department strives to provide nutritional services that optimize the health status of each resident while working within the physician's orders and meeting the resident's individual needs. The meals are provided with alternative choices built into the menus, which are created to enhance resident preference.

Chaplaincy Program

The spiritual needs of the residents are met by the coordination of services of our chaplains that include ecumenical church services, Catholic Mass, individual and groups sessions, Bible study groups, sensory groups, sacraments, end of life support, memorial services, staff support, etc. Churches from the area also provide additional services. Chaplaincy services are available 24 hours a day 7 days a week to residents, family members and staff.

Social Services

The Social Services Department is staffed by full-time, Licensed Social Workers. The department strives to assist each resident and their families in meeting their personal, social, emotional and environmental needs. Discharge planning is coordinated by the Social Worker. The Social Worker also acts as a resident's advocate to ensure that the highest quality of life is met.

Woodside Village

Setting

Woodside Village is a part of 4000 Valley Square, a retirement community that also includes Wheatland Terrace, a 60 unit licensed assisted living facility, Country Estates, a 60 unit senior apartment building, and Town Square, which promotes the warmth of the caring community through its coffee shop, chapel/auditorium, library, and hair salon.

Built in 1999, Woodside Village is designed to maximize the privacy and dignity of the residents we serve. Ninety-six of the rooms are single-occupancy rooms with private bathrooms, and eleven rooms provide accommodations for two people with a shared bathroom.

Care

Though the building's setting is superb, the staff is the greatest asset of Woodside Village. Licensed nurses and certified nursing assistants provide care around the clock to ensure that resident needs are met. The professional interdisciplinary team of caregivers regularly reviews the physical and emotional status of each resident and schedules care conferences with residents and families. Medical care is primarily the reason people move into Woodside Village, and quality improvement processes are in place to ensure the quality of the care.

While medical care is extremely important, there is a loss of autonomy with moving to a nursing home that medical care cannot fully address. To combat the feelings of loneliness, helplessness, and boredom, Woodside Village is pursuing a philosophy of care that commits to creating a resident-centered community where life revolves around close and continuing contact with plants, animals, and children. Woodside Village is striving to provide companionship, opportunities for residents to care for others, and bringing variety and spontaneity that makes it more of a home for the residents. The Hearthstone: A Special Care Unit for People with Dementia

The Hearthstone is a part of Woodside Village. Designed as a safe environment to support the changing needs of those diagnosed with dementia or Alzheimer's disease, the Hearthstone provides care for 28 individuals. Rooms are designed to maximize each person's functional abilities with emphasis placed on individual protection and security.

In the center of The Hearthstone is a large common area equipped with a full kitchen, a large living room with a piano and fireplace, and access to the outdoor landscaped courtyard. The living areas are designed to enhance the feelings of warmth and coziness. Living rooms are available for small groups of residents and families to gather.

The staff in the Hearthstone has special training in the area of dementia care. This training along with the physical design empowers residents to maximize their independence in a safe environment.
